New Patient Paperwork

We want you to have the best experience possible with OPTI. You can help us with this by completing the paperwork for your first appointment before you arrive. If you take the time to print and fill out the new patient paperwork your arrival time for your first appointment only needs to be 5 – 10 minutes prior to your scheduled appointment time. If you need the time to fill out the new patient paperwork please arrive 15 – 20 minutes prior to your scheduled appointment time.

Please bring a copy of your PT order from your doctor, your photo identification and your insurance cards. If you have a co-pay for physical therapy, you will need to pay the specified amount at the time of service.
